If you need a high school transcript from 1995 to the present:

Summit Learning Center: Please email Joan McMillan at [email protected] or you can call Joan McMillan at 708-371-1986 ext. 7000 for information.

OR - send a check or money order for $2 for each transcript with a written request with:

Your printed name (First and Last and maiden name if applicable),

Year of High School graduation,
Copy of a legal form of photo identification (State ID, Drivers License, Passport)
Name and address of the school/business where you want the transcript to be sent

If you need a high school transcript prior to 1995, please call 708-424-2000 and request information through our Administrative Offices.
