Attention Incoming Freshmen

The School Code of Illinois requires that each student entering 9th Grade have a physical examination completed by a licensed physician, advanced practice nurse or physicians' assistant.  Class schedules will not be issued without a completed physical and up to date immunization record on file with the school nurses.  Parents/Guardians must complete and sign the Medical History section as well as the Emergency Information section.  Please turn in the completed physical to the school nurses office as soon as possible.  The physical form below may be printed out.

 

Attention Seniors (12 Graders)

 

Meningococcal Conjugate Vaccination (MCV4) is a requirement for all 12 grade students in Illinois.  Your student must show proof of having received 2 doses of MCV, unless they have received their first dose after 16 years of age.  Please provide documentation from the physician's office as soon as possible.  No schedules will be provided to 12 grade students until this requirement is met.

State law requires the Illinois High School Association to post a hands-only cardiopulmonary resuscitation and automated external defibrillators training video on its website. The law also requires the District to notify staff members and parents/guardians about the video. You are encouraged to view the video, which will take less than 15 minutes of your time, at:
